A - DIET

I. Milk and banana diet for three to five days.

II. Thereafter, the following diet may be adopted:-

1. Upon arising: 25 black raisins soaked overnight in water along with the water in which they are soaked and also water kept overnight in copper vessel.

2. Breakfast: Fruits such as bananas, mangoes, musk melon, custard apple, papaya and figs and a glass of milk. Avoid sour fruits .

3. Mid-morning: Carrot juice or coconut water.

4. Lunch: Steamed vegetables, whole wheat chappatis or rice.

5. Mid-afternoon: Few dates and milk.

6. Dinner: Lightly cooked vegetables, whole wheat Chappatis and fruits.

NOTE: Introduce gradually raw vegetables and sprouted green gram .

7. Before retiring: A glass of milk.

AVOID: Tobacco, alcohol, flesh foods, coffee, tea, condiments, all chillies, white sugar, white flour and their products, fried and greasy foods.

IMPORTANT: Take low salt diet, small and frequent meals, avoid too hot or too cold foods and drink eight glasses of water everyday.

B - OTHER MEASURES

Tissue plasminogen activator (TPA)

There is opportunity to use alteplase (TPA) as a clot-buster drug to dissolve the blood clot that is causing the stroke. There is a narrow window of opportunity to use this drug. The earlier that it is given, the better the result and the less potential for the complication of bleeding into the brain.

Present American Heart Association guidelines recommend that if used, TPA must be given within 4 1/2 hours after the onset of symptoms. for patients who waken from sleep with symptoms of stroke, the clock starts when they were last seen in a normal state.

TPA is injected into a vein in the arm but, the time frame for its use may be extended to six hours if it is dripped directly into the blood vessel that is blocked requiring angiography, which is performed by an interventional radiologist. Not all hospitals have access to this technology.

TPA may reverse stroke symptoms in more than one-third of patients, but may also cause bleeding in 6% patients, potentially making the stroke worse.

For posterior circulation strokes that involve the vertebrobasilar system, the time frame for treatment with TPA may be extended even further to 18 hours.

Heparin and aspirin

Drugs to thin the blood (anticoagulation; for example, heparin) are also sometimes used in treating stroke patients in the hopes of improving the patient's recovery. It is unclear, however, whether the use of anticoagulation improves the outcome from the current stroke or simply helps to prevent subsequent strokes (see below). In certain patients, aspirin given after the onset of a stroke does have a small, but measurable effect on recovery. The treating doctor will determine the medications to be used based upon a patient's specific needs.

Managing other Medical Problems

Blood pressure will be tightly controlled often using intravenous medication to prevent stroke symptoms from progressing. This is true whether the stroke is ischemic or hemorrhagic.

Supplemental oxygen is often provided.

In patients with diabetes, the blood sugar (glucose) level is often elevated after a stroke. Controlling the glucose level in these patients may minimize the size of a stroke.

Patients who have suffered a transient ischemic attacks, the patient may be discharged with blood pressure and cholesterol medications even if the blood pressure and cholesterol levels are within acceptable levels. Smoking cessation is mandatory.

Rehabilitation

When a patient is no longer acutely ill after a stroke, the health care staff focuses on maximizing the individuals functional abilities. This is most often done in an inpatient rehabilitation hospital or in a special area of a general hospital. Rehabilitation can also take place at a nursing facility.

The rehabilitation process can include some or all of the following :
  1. speech therapy to relearn talking and swallowing;
  2. occupational therapy to regain as much function dexterity in the arms and hands as possible;
  3. physical therapy to improve strength and walking; and
  4. family education to orient them in caring for their loved one at home and the challenges they will face.
The goal is for the patient to resume as many, if not all, of their pre-stroke activities and functions. Since a stroke involves the permanent loss of brain cells, a total return to the patient's pre-stroke status is not necessarily a realistic goal in many cases. However, many stroke patients can return to vibrant independent lives.

Depending upon the severity of the stroke, some patients are transferred from the acute care hospital setting to a skilled nursing facility to be monitored and continue physical and occupational therapy.

When there is spitting of blood the patient should keep very quiet. The spitting of blood is often caused by the patient's lifting something heavy or exercising too vigorously. If there is spitting of a large amount of blood, cloths wet in ice water should be applied to the front of the chest. The cloths should be re-wet frequently in order that they may be kept constantly cool. If no ice can be obtained the cloth may be wet with cool water and then held by two of its corners and swung back and forth in the air a few times. This will make it very cool. Be careful not to chill the body. When one is ill with tuberculosis or any other wasting illness, one must be careful not to carry out too vigorous treatment.

Treatment for Hypoglycemia

Management of hypoglycemia involves immediately raising the blood sugar to normal, determining the cause, and taking measures to hopefully prevent future episodes.

The blood glucose can be raised to normal within minutes by taking (or receiving) 10-20 grams of carbohydrate. It can be taken as food or drink if the person is conscious and able to swallow. This amount of carbohydrate is contained in about 3-4 ounces (100-120 ml) of orange, apple, or grape juice although fruit juices contain a higher proportion of fructose which is more slowly metabolized than pure dextrose, alternatively, about 4-5 ounces (120-150 ml) of regular (non-diet) soda may also work, as will about one slice of bread, about 4 crackers, or about 1 serving of most starchy foods. Starch is quickly digested to glucose (unless the person is taking acarbose), but adding fat or protein retards digestion. Symptoms should begin to improve within 5 minutes, though full recovery may take 10–20 minutes. Overfeeding does not speed recovery and if the person has diabetes will simply produce hyperglycemia afterwards.

If a person is suffering such severe effects of hypoglycemia that they cannot (due to combativeness) or should not (due to seizures or unconsciousness) be given anything by mouth, medical personnel such as EMTs and Paramedics, or in-hospital personnel can establish an IV and give intravenous Dextrose, concentrations varying depending on age (Infants are given 2cc/kg Dextrose 10%, Children Dextrose 25%, and Adults Dextrose 50%). Care must be taken in giving these solutions because they can be very necrotic if the IV is infiltrated. One situation where starch may be less effective than glucose or sucrose is when a person is taking acarbose. Since acarbose and other alpha-glucosidase inhibitors prevents starch and other sugars from being broken down into monosaccharides that can be absorbed by the body, patients taking these medications should consume monosaccharide-containing foods such as glucose tablets, honey, or juice to reverse hypoglycemia.

  • Eat a consistent, well-balanced diet that is high in fiber, low in saturated fat, and low in concentrated sweets.

  • A consistent diet that includes roughly the same number of calories at about the same times of day helps the healthcare provider prescribe the correct dose of medication or insulin.

  • It will also help to keep blood sugar at a relatively even level and avoid excessively low or high blood sugar levels, which can be dangerous and even life-threatening.
Exercise: Regular exercise, in any form, can help reduce the risk of developing diabetes. Activity can also reduce the risk of developing complications of diabetes such as heart disease, stroke, kidney failure, blindness, and leg ulcers.

  • If the patient has complications of diabetes (eye, kidney, or nerve problems), they may be limited both in type of exercise and amount of exercise they can safely do without worsening their condition. Consult with your health care provider before starting any exercise program.
Alcohol use: Moderate or eliminate consumption of alcohol. Try to have no more than seven alcoholic drinks in a week and never more than two or three in an evening. One drink is considered 1.5 ounces of liquor, 6 ounces of wine, or 12 ounces of beer. Excessive alcohol use is a known risk factor for type 2 diabetes. Alcohol consumption can cause low or high blood sugar levels, nerve pain called neuritis, and increase in triglycerides, which is a type of fat in our blood.

Smoking: If the patient has diabetes, and you smoke cigarettes or use any other form of tobacco, they are raising the risks markedly for nearly all of the complications of diabetes. Smoking damages blood vessels and contributes to heart disease, stroke, and poor circulation in the limbs. If someone needs help quitting, talk to a healthcare provider.

Self-monitored blood glucose: Check blood sugar levels frequently, at least before meals and at bedtime, and record the results in a logbook.

  • This log should also include insulin or oral medication doses and times, when and what the patient ate, when and for how long they exercised, and any significant events of the day such as high or low blood sugar levels and how they treated the problem.

  • Better equipment now available makes testing blood sugar levels less painful and less complicated than ever. A daily blood sugar diary is invaluable to the healthcare provider in seeing how the patient is responding to medications, diet, and exercise in the treatment of diabetes.

Treatment for Pulmonary Tuberculosis

The goal of treatment is to cure the infection with drugs that fight the TB bacteria. Treatment of active pulmonary TB will always involve a combination of many drugs (usually four drugs). All of the drugs are continued until lab tests show which medicines work best.

The most commonly used drugs include:

* Pyrazinamide
* Ethambutol

Treatment of Hypogonadism in Men with HIV Infection

Introduction: A number of endocrine abnormalities have been reported in people infected with HIV. Among these, hypogonadism is well described, particularly as men progress to AIDS and in those who are wasting. Testosterone has both anabolic and androgenic effects. Although there has been a study showing improvement in quality of life in HIV-positive men treated with high doses of Testosterone, there is no data confirming its usefulness as an anabolic agent. The recommendation is that Testosterone should not be used unless hypogonadism is proven.

Diagnosis: Although some patients present with classical signs and symptoms of androgen deficiency, many have less specific complaints. Symptoms may be particularly difficult to notice in patients with AIDS. Complaints may include decreased libido, erectile function, fatigue, depression, decreased frequency of shaving, and decreased bone density. Laboratory tests should include a total and free (or bioavailable) testosterone, FSH and LH.

Intramuscular Testosterone: Oral administration of testosterone is not practical and is generally not available in the US. The most commonly used testosterone replacement is intramuscular depot injections. This route has the advantages of ensuring delivery to the patient without concerns of patches slipping off or doses being forgotten. The drug itself is inexpensive. A significant problem is the rapid serum peak to supraphysiologic levels, reached between hours to a few days after injection of testosterone enanthate, then a gradual decline over the following weeks. There is no circadian variation in delivery.

Optimal dosing appears to be 200 mg IM testosterone enanthate every two weeks. Injections can be painful, especially in wasted patients, and usually require an office visit. Additional side effects, common to all forms of androgen replacement, may include acne and gynecomastia. Salt retention may be a problem in patients with preexisting congestive heart failure, cirrhosis or nephrosis.

Testoderm scrotal patch: The TestodermR Testosterone Transdermal System is placed on clean, dry scrotal skin. The dosage is a 6 mg patch which is applied at 9 a.m. and worn for 22 to 24 hours each day. The scrotal hair must be dry-shaved for optimal contact; chemical depilatories should not be used. Men may be required to change to close-fitting underwear, and exercise and hot weather may cause the patch to come off. Follow-up bloods for total testosterone should be obtained 2-4 hours after patch placement 4-6 weeks after continuous therapy. If adequate levels are not reached by the end of 2 months, another form of therapy may be required.

In long term studies of hypogonadal HIV negative men, 60% maintained normal serum testosterone levels. Serum dihydrotestosterone (DHT) levels were elevated in all patients who had increases in serum testosterone levels.The significance of the increased DHT is unknown. Self report of mood, energy level and frequency of sexual thoughts and intercourse all increased over baseline, with nearly 50% of the patients reporting increases by week three.The most common side effect was local skin problems.

Androderm nonscrotal patches: Androderm R is a permeation-enhanced testosterone transdermal system that is applied to nonscrotal skin. Again, all studies were conducted on HIV negative, hypogonadal men, with no published literature on the use of Androderm in HIV positive subjects. Dosage is two patches, applied nightly in a rotating manner on the back, abdomen, upper arm or thigh.

In three clinical trials of 121 hypogonadal men, 92% of subjects achieved normal morning testosterone levels and 88% bioavailable testosterone. Pharmacokinetic studies show a steady testosterone level in a pattern mimicking the normal circadian.

Self-reported symptoms of impotence, decreased libido, fatigue, hot flashes and depression all improved. No changes were noted in aggression or hostility. In previously nonvirilized subjects treatment led to virilization and increases in lean body mass.

The most common adverse event was local skin irritation. In long term follow-up, 53% noted some form of local reaction at the application site. Of 121 men treated, 12 patients (9.8%) discontinued therapy due to a site reaction. Pruritus is the most common reaction reported (37%), followed by local blister formation (12%) or erythema (7%).

Contraindications to testosterone therapy: Androgen replacement is contra-indicated in men with known or suspected breast cancer, prostate cancer or Kaposi sarcoma. Fertility will decrease during testosterone treatment. Decreases in serum HDL, gynecomastia, and increases in prostate size are similar in all forms of TRT and generally not clinically significant.
