Tronco encefálico

El tronco encefálico, o tallo cerebral, es la parte del sistema nervioso central compuesta por el bulbo raquídeo, protuberancia anular y el mesencéfalo (pedúnculos cerebrales, cuerpos cuadrigéminos). Es una vía de comunicación entre la corteza cerebral, la médula espinal y los nervios periféricos. El tronco encefálico controla varias funciones que incluye la respiración, regulación del ritmo cardíaco y aspectos primarios de la localización del sonido.

Núcleos Vestibulares

Los núcleos vestibulares son los núcleos craneales del nervio vestibular. Dentro del tronco del encéfalo hay cuatro núcleos vestibulares: 1) núcleo vestibular superior, que se encuentra localizado en la protuberancia anular; 2) núcleo vestibular lateral, ubicado en la médula espinal; 3) núcleo vestibular medio, en el piso del cuarto ventrículo; 4) núcleo vestibular inferior, ubicado también en la parte superior de la médula espinal.

Los núcleos vestibulares reciben señales de la porción vestibular del VIII nervio craneal. Los axones en el nervio vestibular son prolongaciones de neuronas situadas en el ganglio vestibular de Scarpa, el cual yace en el meato auditorio interno. Estos axónes centrales terminan en los núcleos vestibulares y el cerebelo. Los axones periférico de estas células nerviosas reciben señales de los receptores del laberinto vestibular (células filiformes).

Formación reticular

La formación reticular es un conjunto de núcleos grisis interconectados por fibras nerviosas que se encuentran en la protuberancia anular del tronco del encéfalo. La formación reticular tiene como función la regulación de los ciclos circadianos de sueño-vigilia, además de identificar estimulaciones de fondo irrelevantes. Estroma corneal

El estroma corneal es una capa de tejido fibroso, duro y transparente que se encuentra por debajo del epitelio corneal y la membrana de Bowman, los cuales también son componentes de la córnea del ojo humano. Neuropatía óptica isquémica

La neuropatía óptica isquémica es un daño en el nervio óptico causado por una obstrucción en los vasos sanguíneos que lo irrigan. Esta enfermedad oftalmológica puede provocar una ceguera permanente si el daño en el nervio óptico es irreparable, pues es el nervio que transmite las imágenes que se graban en la retina hacia el lóbulo occipital. A - DIET

I. Milk and banana diet for three to five days.

II. Thereafter, the following diet may be adopted:-

1. Upon arising: 25 black raisins soaked overnight in water along with the water in which they are soaked and also water kept overnight in copper vessel.

2. Breakfast: Fruits such as bananas, mangoes, musk melon, custard apple, papaya and figs and a glass of milk. Avoid sour fruits .

3. Mid-morning: Carrot juice or coconut water.

4. Lunch: Steamed vegetables, whole wheat chappatis or rice.

5. Mid-afternoon: Few dates and milk.

6. Dinner: Lightly cooked vegetables, whole wheat Chappatis and fruits.

NOTE: Introduce gradually raw vegetables and sprouted green gram .

7. Before retiring: A glass of milk.

AVOID: Tobacco, alcohol, flesh foods, coffee, tea, condiments, all chillies, white sugar, white flour and their products, fried and greasy foods.

IMPORTANT: Take low salt diet, small and frequent meals, avoid too hot or too cold foods and drink eight glasses of water everyday.

B - OTHER MEASURES

1. We packs for one hour in the morning and evening on an empty stomach, hot packs in case of abdominal pain.
2. Yogasanas like uttanpadasan, pavanmuktasan, bhujan¬gasan and shavasan.
3. Avoid stress, nervous strain, and worries.

Queratectomía fotorrefractiva

La queratectomía fotorrefractiva es una cirugía de laser utilizada en oftalmología para tratar los defectos de refracción del ojo, como miopía e hipermetropía, para evitar el uso y dependencia de anteojos o lentes de contacto. Por medio de la queratectomía fotorrefractiva, usando el laser de excímeros, se cambia la forma de la parte anterior de la cornea en forma permanente, removiendo por vaporización una pequeña cantidad del estroma corneal, previa ablación del epitelio corneal. Durante la operación un sistema de computadora rastrea la posición del ojo entre 60 y 4.000 veces por segundo, redireccionando los pulsos laser para poder realizar una cirugía precisa.

El epitelio de la cornea es suave y de rápido crecimiento, pudiendo regenerarse por si mismo por medio de las células madres corneales en el lapso de unos pocos días sin pérdida de transparencia. En cambio el estroma de la cornea tiene una capacidad de regeneración muy limitada. Es por ello que cuando se le cambia la forma por medio de una queratectomía, el estroma no volverá a retomar su estructura anterior causante de la miopía. En cambio, el epitelio corneal que se retira o ablaciona para poder cambiar la forma del estroma, se regenera rápidamente.

Trabeculectomía

En oftalmología, la trabeculectomía es un procedimiento quirúrgico aplicado en el tratamiento del glaucoma para aliviar la presión intraocular quitando parte de la red trabecular del ojo y estructuras adyacentes. Es la cirugía más común para el tratamiento del glaucoma. La trabeculectomía permite el drenaje del humor acuoso desde dentro del ojo hacia la parte inferior de la conjuntiva donde es absorbida.

Este procedimiento se lo realiza comunmente bajo anestesia monitoreada utilizando un block retrobular o peribulbar, o una combinación de anestesia topica y subtenoniana. Para llevar a cabo una trabeculectomía se realiza una perforación en la porción más externa del ojo o esclerótica, hasta llegar a un espacio del interior del órgano de la visión que se llama cámara anterior. Por este nuevo conducto creado por la cirugía se consigue que drene hacia el exterior el humor acuoso.

Red Trabecular

La red trabecular es un área de tejidos esponjosos ubicados alrededor de la base de la córnea, cerca del cuerpo ciliar. La función de la misma es el drenaje del humor acuoso desde la cámara anterior del ojo hacia el Canal de Schlemm que finalmente desembocan en el sistema sanguíneo. El tejido esponjoso que forma la red trabecular está recubierto por células especiales llamadas trabeculocitos.

La red trabecular está compuesta de tres partes: 1) la red uveal interna, que es la más cercana al ángulo de la cámara anterior y que contienen trabéculas o tabiques orientados en forma radial que encierran pequeños espacios internos; 2) red corneoescleral, la cual tiene gran cantidad de elastina (proteina con funciones estructurales) en forma de láminas delgadas perforadas; 3) tejido yuxtacanalicular, que yace en forma adyacente al canal de Schlemm y está compuesto de tejido conectivo.

La red trabecular desempeña un papel importante, ya que evita el aumento anormal de la presión intraocular, lo que puede provocar un glaucoma. Esto se debe que a través de ella fluye el humor acuoso hasta que finalmente es drenado al sistema venoso por el Canal de Schlemm. Cuando la red trabecular no cumple esta función de drenaje, se produce un aumento de la presión intraocular del humor acuoso.
